What sort of questions would you put on a questionnaire for someone who is looking to adopt from you?

Have they had rabbits before? What did they pass away from?
Have they got bunnies now? What age/are they neutered?
Vets name/address
accommodation
do they have permission from landlord
What plan to feed
what do they expect from rabbit/why chosen rabbits

Have they done any research on rabbits?
What vetrinary (can't spell) arrangements have they got? I mean, do they realise that vaccinations need to be kept up and they need to see a rabbit savvy vet?

I think a questionnaire is a really good idea.

what are the most common ailments a bun can suffer from? What size accomodation does a bun need? Is a bun better alone or in a pair? What are the basic dietary requirements of a bun?
