Help re fur loss

I have always had a problem with Poppy's dirty bum, which I have to wash frequently. Tried lots of different ways of cleaning it, but dunking her in shallow water is the only thing that works. Anyway last week I noticed that she has lost all her fur around tail and legs. I put some dermisol cream on it, then took her to vet. She said there was nothing much I could do except carry on putting on dermisol. But it is not getting any better and her poor little legs look so pitiful, I feel there must be more efficient treatment. Has anyone had this problem and found a remedy that works?
 
think I would be inclined to ask another vets opinion on this;) there a quite a few reasons why a bun would have fur loss like this, one thing you need to look at is her diet for the sticky bum, but also, she could have a urine infection which could be resulting in urine scald, or, the onset of arthritus which again could mean that she is sitting in her own wee, there are a few that could be causing this. Have you asked on here for recommendations in your area for a rabbit savvy vet;) ?
