for me it is when a day has more 'bad' times than 'ok' times
when the rabbit ceases to show interest in life around them and/or their companion(s) (if they have one/any)
if the rabbit finds any essential hands on care very distressing, especially if the 'hands on care is needed multiple times a day. some rabbits tolerate this really well, others become more and more stressed,
dreading the next time they have to be picked up and never able to fully relax as a result
i do not take 'he/she is still eating' as being a sign that they still want to go on. imo in general i feel that there has to be more to their day to day life than that.
also, look into the rabbits eyes, they will tell you xx
