Messy Bottom

My bun constantly has a messy bottom, we took him to the vets when he was small up until now and its general laziness that he gets it all the time.
I've just cleaned him up again today but he got a bit fidgety so i couldn't get it all. I got most of it off but there was some around his "man part" still but because he was really fidgety i had to put him down.

Would it be best to leave him to clean it himself, or try again? Hes happy at the moment bouncing around the living room, i don't really want to stress him out.

Any help would be appreciated xx

What is his daily diet ?

You do need to clean him up thoroughly as he is at high risk of Flystrike

I give him unlimited hay, tiny handful of pellets and some greens. I've been to vets so many times over the 4 years i have had him and they say he's perfectly fine, just lazy. Ive got the majority of it off, there's just a little bit left
 
Is your vet rabbit savvy? If your bun doesn't eat his cecals it's unlikely to be lazyness. It might be worth cutting out the greens, back on the pellets and upping the hay.
Is he an indoor or outdoor bun?
