New owners should be asked to sign an adoption form. This records information about the new owner, their responsibilities and the rabbits details. You should retain one copy of the adoption form and the new owner should keeo the other.
The adoption form should include the following information:
Full details of the rabbit including name, age, sex, description and reference number.

Agreement that should the rabbit become sick that a qualified vet will be consulted. That should the owners circumstances change, and they be unable to keep the rabbit, that the rescue will be the first place they call to discuss options.

that one's not always easy if you use 4 different vets for various things as we do!..
This is ours:
In accordance with our wish to ensure that all of our rabbits find suitable homes with responsible owners we operate a Contract of Return. This means that should you find yourself in a position where you can no longer care for your rabbit for any reason the rabbit in question must be returned to the rescue for re-homing. Furthermore we must accept back any rabbit which we have previously re-homed subject to the conditions that:
1. It can be proved beyond reasonable doubt that the rabbit being returned was the same rabbit we re-homed.
2. The rabbit is not being returned in a state where the only reasonable option will be for us to have it euthanised.
3. The rabbit is not being returned in a state where its condition will affect the health of other animals at the rescue.
4. We have the ability to provide the rabbit with accommodation and a reasonable quality of life until such time that we can find it a new home.
We also retain the right to demand the return of any rabbit that we have re-homed should it be found in any of the conditions listed below:
1. The rabbit is living in inhumane conditions whereby it is suffering a diminished quality of life.
2. The rabbit requires veterinary treatment, which is being denied and thus causing pain and unnecessary suffering.
3. The rabbit is enduring deliberate cruelty or neglect.
4. The rabbit is being irresponsibly bred.

We provide a seperate medical record for details of vaccinations and neutering etc.
Ours contains a neutering clauseng as on occasion we let young rabbits go prior to neutering and there are always those who may not pass the pre-op check due to heart conditions etc.

When I adopted two of my rabbits I also had to sign that if one of the bonded pair died, I would commit to finding another partner for the rabbit, returning him/her to the rescue or for the remaining bun to become a houserabbit. I forget the exact wording but it was designed to ensure that rabbits weren't kept alone outside.
The form also stated minimum accommodation sizes so that the adopter knew exactly what the requirement is if changing the accommodation, and the need to notify the rescue if the adopter moved house.

the hopper haven terms and conditions of adoption are a full A4 size :shock:
by signing the adoption form i agreed to, provide the care as required by RWA leaflets, vaccinations, vet treatment at the first sign of illness, if one dies a companion must be found, the animal still belongs to happer haven and must be returned to thier care should my circumstances change, it also specifies the minimum living space.
theres also a guinea pig part too :)

The other thing I would suggest is getting a solicitor to look over your forms, we have had ours checked by a friend.

Sadly..even if checked by a solicitor adoption forms are not legally binding:cry:

No I know that, but they help a lot with the wording to make them more 'official' and therefor scary! :lol:
I am quite interested to see other examples with the aim to improve ours as necessary.

I thought all rescues passed on the vaccination card to the new owner, do not all do this then? I have mine from Tracy and it has her vets card clipped to it so should my vets want to contact Simon they can:)
My vets send me cards when their vaccinations are due, does anyone else's vet do the same?

I get a postcard from my vets when mine are due.
I guess there are some circumstances when there might not be vacc cards eg the rescue vaccinated themselves or vaccs haven't been done so it's a get it done by date.
So, there should be something with the adoption paperwork (date on form or vacc card) that indicates current vaccination status and due dates.

We send the vacs card..if one hasn't been provided we put date vacs done on adoption form and vet details can be provided..
Sooz..yep I too am looking to improve our forms!:D
